Word 2000 Creating an Index

I would like to create an index on my very large word 2000 file using an Accordance file. I understand that the Accordance file is a 2 column table. Can someone please give me an example of how I would create the Accordance file to give me the following results:

Creating
Creating a Shortcut Icon ........ 7
Creating a New Password ......... 8
Creating a New Person ........... 12

G
----------------
GHHS
Logging into GHHS................ 6
What is GHHS .................... 4

Sorry. It's not an accordance file. It is called a Concordance file.
