Windows Folder Icons, in other applications not showing

Hi
when i click save as or save link or something and the little folder preview comes up, asking where to save
none of the folders in there have their icons, or actually preview the things thats in the folders either...
noticed that its happening in firefox... and also itunes when i say add file and the little folder pops up same deal... here is a preview

also when i actually go into the folder that should display the same, the icons have their picture

it seems to be only in external programs that a folder option to save or import something etc in... i've tried rebuilding the icon cache thingy and also running in safe mode but the problem still occurs

Have you tried selecting a different view

using the downarrow to the left of the ? at the righthand end of the toolbar

no views fix it... i've seriously felt like i've tried everything... cleaning the registry, rebuilding the iconcache deleting rebuilding the iconcache

nothing is working

you can see from the image that its not just the icons of files in the folders, but also the actual folder paths on the left hand side that don't show either
