Why my PC Freezes in every few minutes?

My PC freezes in every few minutes, stays like that for 2-3 seconds and then works fine again. The mouse cursor also do not move at that time. This happens in every 10-12 minutes. Does not matter what program I am using or watching movies this happens in every 10-12 minutes.

I have 2 operating system, Win 8.1 and Win 7. This problem happens in both operating system.

I have 2 Hard Disc, Samsung 500 GB Hard disc, Toshiba 1 TB. Both operating systems are in the 1 TB Hard disc.

I also got a "Windows Detected a hard Disc problem" in every few hours. The problem is in the Samsung HDD. I tried to run my PC without the Samsung 500 GB but the freeze problems still remains.

I have a and using this PC for around 5 years. Do I have to chage both my Hard Discs or any other Hardware? Please help me.

I have
Core i3, 2.93 GHz,
4GB DDR3 RAM
1500 GB Hard disc (1 TB + 500 GB)
1 GB Geforce GT 620 Graphics

Run a chkdsk from each of those OS's.
Check the output log to see what it found.

Visit each hdd maker's site run their drive fitness test.

May there are some viruses in your machine so scan it.
