Why can't I be my own VoIP vendor? What's in the way?

I am curious if anyone is familiar enough to know what is stopping an individual from becoming their own VoIP provider? How does one associate a telephone number from the PSTN with an IP address? It would seem that the price of equipment has gone down enough to begin considering this. If I can buy a $500 router, and be done with it, why not....
