Who can name this old school media?

A little off topic but bear with me. I'm in the middle of a project and need the older people's help. This:

Is an old school laser disc that I am having trouble finding. The man who has it claims it to be an Optimem disc. Trick is for those that were around then and have seen these in person will know that there is a plastic casing over it much like a floppy and this is merely the inner disk. Hints are that it is 12" and is made of glass unlike some later plastic laser disks. If still unknown please offer suggestions on close look a likes. Has to be glass though.

And the outer ring gives me a clue it may be that old Next Cube optical media.

I searched for a picture.but came up short. I know the technology but it is not something used outside of legacy systems.

I was assigned to be the tech writer for a laser disk that was being built for use on large computer systems.
The project never was completed but they were using media from Sony.
It was glass and was coated with gold. There was an "overcoat" of some type of clear plastic.

It laser disk drive never made it as far as production.
