Any application that supports forms should suffice for this. You've got ample choice.
In MS Office we have: MS Word, MS Excel, MS Access, MS Outlook, MS InfoPath. tells about MS Word.

But - apart from the forum you post in - there's nothing that says you want to limit yourself to MS Office. So let me also suggest: Adobe Acrobat, html and Visual Basic.

It all depends on what you want to DO with the data typed in by the user.
