I can not get pass the blue "Welcome" screen without hitting ctrl-alt-del. The "require users to enter ctrl-alt-del when logging in" option is already disabled.
Any suggestions?

I think some cleaning is in order. Try disabling some start-up scripts, they might be causing this problem. CCleaner is a good freeware for a total system cleanup. Keep some space on the C: drive. I hope it helps.

I rarely encounter this unless there were pests cleaned off or the other usual of registry cleaners (nice way to mess up a good PC.)
