webcam streaming video

I would like to implement live audio and video, just like on home makeover, so that I can see and talk to family members or who else same a like setup...

I have windows XP as an operating system on two computers and a webcam on each computer. when the camera is tested you can see the real time image on the screen and the audio test provides a beep sound as it is supposed to. I just can't get the computer to talk to each other...
Can you provide any help?
