TOC generates field codes

When I generate a Table of Contents or an Index, instead of creating the TOC or Index, Word places field code text in the document. An example of the result is:

{INDEX \c "2"\z"1033}

Looks like I may have accidentally done something to have effected this change. What can I do to revert back and get an actual TOC or Index generated?

Have a look at Tools>Options>View. Especially the 'show field codes' checkbox might be interesting.

Or - if that doesn't help - press shift-f9 on the field code to toggle between code view and normal view for that particular field.
