Time machine problems

My wife and I both are using (or thought we were) Time machine. I had my HD crash and when I went to get my backups they were not there. My wife checked her TM and it stopped backing up 3 weeks ago even though she has changed many files. What is the problem?

was there any notification that it had stopped?

a change in the menu bar icon perhaps?

Is the drive you back up to, still functional?

Is it always connected and powered on/

I received a message box saying it could not complete the backup. Error creating the backup folder. Always connected, always on.

Sometimes the drive index gets corrupted

or the Backup Database index gets a glitch.

The external drive (LaCie) I'm using is brand new. Apple said it may be the drive, but it seems to be working with Carbon,copy cloner. I hooked up an older WD to TM and so far it is working. It's getting frustrating.
