Text Caption Underneath Field in MS Word

I've created a Word legal document where in the paragraph a person would fill in their name and other information. What I would like to do is have small text underneath the underline that would basically describe the field, like a caption. I'm able to make text subscripted, but cannot get the text to be underneath the line. I'm sure that this can be done but my Google search proved fruitless. Any info would be much appreciated!
