Software for web interface?

Let me see if I can give you a (somewhat) quick synopsis. I am assisting with conducting an survey does not have up to date emails. Thus, we're reaching out to survey respondents via "snail? /WE mail to ask them to go to a website to enter their current email address to participate in our main study (survey). They are provided with an option of whether they would like to participate or not. If they say ?yes?, they will be directed to a page that asks for their current preferred email address. Thus, we will be able to harvest emails of those who want to participate and be able to send follow up emails. This is all done via the survey host site surveymonkey.com.

Here is the tricky part. Once, respondents say ?yes?, the people I?m working with would ideally want the participants who enter their email for participation to be sent an email (immediately or near immediately) with a link to the main survey. Surveymonkey.com can provide the unique URL addresses and the email management system. But the immediate emails are where I?m stuck. Survey monkey, while pretty extensive in its capabilities, doesn?t have the capability to collect emails (into a csv file), reformat the data for input into distribution list in our surveymonkey.com account that immediately uploads the emails for distribution to people who want to complete the survey.

The person on our team, who, in the past, purchased a software (but cannot remember the software?s name) that allowed the 'harvesting' of email addresses, that then put them into a file that they could use in outlook (the email program at the school). It then sent each a survey link immediately to the survey. So, after all that, my question is this: Are you aware of any software that may be able to do this (a macro of some sort)? I realize this is a lot, but anything you may be aware of will be of help to me. Otherwise, I?m reduced to manually harvesting and formatting emails and sending weblinks to survey respondents. Any help would be greatly appreciated.
