Would have me swap out this router for another and see if that's it. I take it you checked all the settings specific to this ISP. What you posted looks OK but as DSL ISP settings change with both ISP and DSL modem I can't check that for you.
Let me preface this by saying that I'm not all that great with tech stuff so it's entirely possible that there's an easy fix I overlooked. Yesterday my internet went down without any provocation so far as I can tell. On the router the power, wireless, and the ethernet ports I had connected at the time were lit up green with the adsl and internet lights off completely. I called the ISP and they say the problem isn't on their end so I restarted the modem/router combination (it's a Netgear N300 Wireless Gigabit ADSL2+ Modem Router DGN3500), unplugged it and plugged it back in both with the computers on and off, tried a different phone jack, tried a different cable, did a factory reset, and called netgear support where they told me they wouldn't help fix it without a $40 payment. After the factory reset I logged into the router manager page and tried to use the setup wizard but it says "The Wizard was unable to establish a connection. Please check the DSL cable before continuing." While fooling around in the router manager the internet light came on red for several minutes before going back off. I figure there's something I can do to fix it on that router manager page but I have no idea where to start. This is what the ipconfig /all says, but I don't know what's relevant:

I'm running windows 8.1, not sure what all other info you need (bank account? Mother's maiden name?) but just let me know. I've only had this router about a year so I'd like to fix it if it's salvageable. Thanks in advance.
