Problems configuring D-Link 300T ADSL modem on Ubuntu 5.10

I have been using this modem for a very long time but in Windows. When I instaled the Ubuntu which is the first Linux OS that I have ever instaled and used I was left with a system that can't connect to the internet. Could someone please tell me how to configure the buntu to connect to the internet by using this modem?
p.s. My ADSL provider has constantly changing IP address and requires user name and passowrd sign in everytie when you want to access the internet.
