There's really nothing wrong with it.. Whenever you change the BOOT SCREEN using Tune-up Utilities, it automatically creates a backup and sets its entry in the "BOOT.INI" file, which you see at the system startup! It's created there so you can use it as a "Restore" option should you ever need to.

If you want to remove the entries, edit C:\Boot.ini file and delete that line. (The "Boot.ini" file is a read-only hidden file)...
