Answer
Reinstall the OS.

There are articles on the web where you lock down Windows and the last step removes the admin account. So if you run across this you reinstall. This is by design so it's no bug.

Answer
If Windows XP, Have You Tried Booting To Safe Mode?
Mar 21, 2015 3:18AM PDT

Unfortunately, you didn't tel us the operating system of the computer in question.. If the computer is using the Windows XP operating system, then simply boot into Safe Mode where it will allow the hidden "administrator" login.. If you're really lucky, then original owner didn't create a password for the "administrator" login, you can leave it blank, then log in.. Once logged in, create a new user profile, give it admin privileges, give it a password, then restart into "normal" Windows.

How To Start In 'Safe Mode'

Hope this helps.
