New Pioneer SC-89 input switching voodoo - mysterious reset

When using a new Logitech universal remote to control either an HDMI equipped cable box (Xfinity/Comcast) or an HDMI connection to an Xbox 360 the audio input of the Pioneer SC-89 initially changes to the proper desired setting then flips to "TV" forcing manual intervention to revert back to the proper setting.

Here is the connection configuration:
- To watch cable on the Samsung 4K UHD - from Pioneer SC-89 "OUT 1 CONTROL" to the TV's HDMI 1 input port. From the Xfinity cable box HDMI to the Pioneer SAT/CBL input port. When activating this mode, everything initially switches properly, then it flips to displaying "TV" on the receiver and you have to manually change back to the SAT/CBL input on the receiver to get the audio and video.

- Playing Xbox ... same issue. Connection from XBox HDMI output to the Pioneer HDMI 4 input. Samsung TV is (same as above) from the Pioneer OUT 1 CONTROL to the Samsung HDMI 1 input. Same behavior. Everything initially switches fine including to HDMI 4 (renamed to Xbox 360 on the receiver) but then it flips to "TV" input. I have to manually revert back to HDMI 4 for proper functioning of the Xbox audio and video.

I do have an external HD over-the-air antenna hooked to the Samsung which shows up as the "TV" input on the Samsung menus but I can't seem to find how/where that translates to the Pioneer SC-89 receiver inputs or if this even factors into the issue I'm having.

Any and all assistance from forum members greatly appreciated.

...everything (e.g. factory reset) and exclude the connected OTA antenna?

My next troubleshooting is to completely disconnect the OTA connections from the whole equation and delete "TV" as an input source from the Samsung.

Something else I've learned that may trigger more insight ... when using the Logitech Harmony remote, and selecting either Xbox or SAT/CBL it still exhibits the behavior mentioned above, meaning it initially switches to the proper HDMI input then reverts to the "TV" input. Using the remotes own capabilities, if I chose "Help" the Harmony remote attempts to resend all the proper settings (HDMI inputs) to the devices (the Samsung TV, the Pioneer ... and the Xbox if that is chosen). That sort of works but interestingly, even though it gets the receiver to the proper HDMI input and the function working, it only deploys the sound in stereo mode using front left and front right speakers. To get either Xbox of CAB/SAT Comcast/Xfinity to leverage full surround system and the FR, FL, Center, RR, RL and sub-woofer, instead of choosing Help from the Harmony remote, you should choose "Fix it myself" and then explicitly re-select the proper HDMI input. This not only gets it working but with the desired 5.1 audio. Unfortunately, even though the remote is programmed properly I still suspect something on either the Pioneer A/V side or the OTA side is screwing up the settings.
