MS Office 2003 Word Question

I am trying to create a permanent document (template?) which has several fields that will change. I want to be able to simply tab to the next field and enter my changes (e.g. date, name, address etc.). Can anybody help me with this? I'm sure it's simple, but I'm at a loss.

Search the Help for Forms. That's what you seem to need.
