making a question sheet

I have office xp. Not a heavy user but I have punched all the buttons I can and am failing to see how to do something.

I would like to make a question sheet that the user can enter the answers.
Similar to this > Name:_____________________

I would like for the user not to be able to access the name space and the underline part. I also would like to be able to tab from one space that needs filled in to the next.

Is there a way to lock the question so to speak and make the tab deal work?

Dang, I looked all over that place looking for something like this.. Now if I could just find one of their templates that shows what they are talking about. so Far no luck there.
