If the stock defrag finishes and you still see a slow machine my bet is it's not the fragmentation slowing the machine. There are some that spend weeks of each month running defrag and we can't help those.
But if you run Windows XP do try flipping the IDE channels to PIO and back to DMA first as well as being spyware and pest free.
Read http://www.microsoft.com/whdc/device/storage/IDE-DMA.mspx on how to flip the modes.
Use EWIDO and HOUSECALL and dump what they find.
I also see you don't have SP2. Very strange.

How do I defragment what seems to be permanently fragmented?
So, as most people seem to endure ? their computers get painfully slow. Regular defragmentation of key drives is usually a healthy activity that keeps the gears running smoothly.
The last time I tried to defragment my C:\ drive I ran into a few issues. The first was that it was 32% fragmented... explaining why I was having trouble getting the computer to successfully wake up from hibernation (though, Windows XP will be slow no matter what.) The second was that after a seemingly quick run at the defragmentation process, it would only drop to a still-painful 24% total fragmentation rate. The result was 7 or 8 files that have several hundred fragments, each that couldn?t be processed. These may have been key windows files.
How do I get my computer up to speed again?
