HELP: Wiring rj45 and rj11 with single Cat5e wire

Newbie here and would appreciate any help! I've updated my networking hardware and now my phone lines are not working.

I have fiber coming to the house going into the service providers modem outside. Previously, I had the modem feeding into an OnQ 10/100 router. One of the ports had a single Cat5e feeding a wall plate in the kitchen that was split between a rj45 jack (orange and green pairs) and a r11 jack (brown pair). Everything worked fine. FYI - I also have a second IP address coming in that feeds the TV cable.

I've updated all my equipment and have replaced the old 10/100 router with a Ubiquiti USG going into Ubiquiti 8-port 150w gigabit switch. From the switch I inserted the previous Cat5e cable going to the kitchen. The ethernet works just fine (although its downgraded to 10/100 because its only using 2 pairs but the phone line now no longer works.

The service provider says that everything looks fine on their end. Any ideas as to what might be going on? The wife is not too happy that I messed up the phone Sad
