What you can do:
1. Scan it into a picture file (.jpg or so).
2. Put it as background picture or watermark in a Word document.
3. Make the Word document into a fill-in form, with fill-in boxes at the right place.
4. Save it as a template.
That's the big picture, and I think it would work. Possibly it can be done in Excel also. http://forums.cnet.com/5208-6129_102-0.html?forumID=29&threadID=199794&messageID=2146256#2146256 is a previous discussion on the subject.

I was given an NCR form to fill out daily for work. I want to be able to fill this out on my computer and email it or print out a hard copy to give to my boss. I have read the forums on this topic and am still somewhat confused as to my basic question which is:
What do I need to do to be able to convert this hard copy form into a template that I can either fill out in Word or as a PDF file so that I can save it each time, and email it or print it out? I have a pc, a scanner, Word and Adobe Reader 8. What is the easiest and least expensive way? Does my scanner have to have particular capabilities. It is an HP7130xi copier/fax/scanner/printer.
Thanks for your help. I appreciate it.
