Flash drive i/o error

Hello.
I have a 4gb flash drive which i cannot format through windows or hp utility. The flash drive gives the i/o error when i try to format or everything. I run WINxp pro i tried in other pc but the same.
I tried the testdisk 6.11 and it can locates the 3 partitions and i tried to analyze them but stucks on 73%
Is it dead?
Any help please?

However, your diagnostics aren't quite clear. You don't format the flash drive, you format a partition. And with 73% OK it could be that 2 partitions are OK and only the third dead.

But it might be best to throw it away anyway. Replacement isn't too expensive.

THe flashkeys are very cheap, i know but it's a good oppurtunity to learn something by restore or fix it but maybe it couldn't be
