fdf...forms...still so confused...

I've tried reading the instructions provided by Adobe on how to make is so that the reader of my PDF can enter his comments, but I'm still so lost. I just want the person that is reading my PDF to be able to write comments after I ask a question or fill out a form and I can't believe that Adobe would make it so difficult as it seems to be.

So how can I create a box that someone can write an answer to a question in such as Name: and then there would be an invisible box or a line that they could write their answers in.

Can anyone give me number step-by-step instructions on how to do this. Thanks.
