Answer
Use GeForce Experience for driver
Oct 19, 2018 10:39AM PDT

I had the same issue last week while playing Battlefield 3. My specs were

Intel Inspiron 15 3000 series
Core i7-5500 CPU @ 2.40Ghz
2 GB Nvidia Graphics Card

The error that I got is

DirectX function “FindClosestMatchingMode" failed (“The video card has been physically removed from the system, or a drive upgrade for the video card has occurred.&rdquoWink GPU: “”, Driver.”37849″.

So what i do is, update my graphics driver using GeForce (*) experience instead of thirdparty tools. now everything works fine.
