Computer Issue (disk 100%)

My computer shows disk 100% in the task manager. It takes a lot of time to boot. I changed my operating system, formatted my computer and even changed my hard disk. The problem still remains. How can I solve this?

Let's check the HDD health with a Web Speccy report (link to follow) but before you do that, turn off this on W10 which makes our drives do more work,.
