I do know that you can go to a computer hardware store like Circuit City and buy CD printer. I think it's about $400.00. It will acutally burn the image on to the disk as opposed to a cheap sticker gig.
For creating the inserts etc.. I would use the Adobe CS2 software. Obviously Photoshop for creating the artwork and then InDesign to create the layout. I know that the spine is simply an extension of the back panel. Here's a free template for the front and back of the CD cover.

You would basically create a new document in InDesign to the sizes for front and back. Note that they're not the same sizes. The back panel of the CD cover will be a little longer than the front cover.
Once you've got the dimensions right just layout the artwork and typography and you're good to go.
Hope this helps.
We're looking for help in finding the best software to use to create professional CD inserts. We're artist/photographers looking into designing CD covers and spine jewelcase inserts as a business for bands and musicians. We also need a template for disc design, specifically NOT the sticker type, rather printer-ready templates.
Any input would great.
Thanks.
