Background loads, icons don't

I thought this started before before installing SP2. Since installing, every time I boot my desktop background loads with no icons, start button etc. If I hit my reboot button, it usually comes up okay. When I use alt+ctrl+del and log off the user (I'm the only user on my home edition) then log on, the icons are always there. How can I avoid going through these contorsions?

When you are able to log on, run all the viruses and spyware test shown in the following Antiparasite Tools message.

Thanks. I already ran spybot s&d (several times) - DSO Exploit keeps returning. I also run Adaware. I'll try some of the others.
